NCP: Nutrition Monitoring and Evaluation
The Academy's nutrition care process for the profession has four steps - (1) nutrition assessment, (2) nutrition diagnosis, (3) nutrition intervention, and (4) nutrition monitoring and evaluation.
Nutrition Monitoring and Evaluation, the fourth step, identifies patient/client outcomes relevant to the nutrition diagnosis and intervention plans and goals. Nutrition care outcomes -- the desired results of nutrition care -- are defined in this step. The change in specific nutrition care indicators, though assessment and reassessment, can be measured and compared to the partient/client's previous status, nutrition intervention goals, or reference standards.
